The New Choice of Tacrolimus in the Treatment of Refractory Ulcerative Colitis

چکیده

Ulcerative colitis (UC) is a chronic non-specific inflammatory bowel disease with lesion confined to the colonic mucosa and sub mucosa [1]. According to the activity degree, UC can be clinically classified as mild, moderate and severe refractory diseases. Aminosalicylates and corticosteroids are traditional medicines used for mild and moderate UC, respectively, while immuno-suppressants (e.g., methotrexate and cyclosporine A) are generally used for severe refractory UC [2,3]. Unfortunately, approximately 30% of UC patients eventually require surgery after treating with these traditional medicines, which are mainly due to their gastrointestinal reaction, renal toxicity and neurotoxicity [4]. Therefore, there is an urgent need to develop a novel medicine to improve the outcomes in patients with UC. Tacrolimus that targets a specific immunological pathway has been studied as a promising UC therapeutics [5,6]. This review will provide a brief overview of several up to date clinical studies on tacrolimus in UC treatment.

Economic Evaluation of Infliximab for Treatment of Refractory Ulcerative Colitis in Iran: Cost-Effectiveness Analysis

The aim of this study is to assess cost-effectiveness of infliximab, compared with conventional treatments in patients with moderate to severe Ulcerative Colitis (UC) in Iran. We developed an analytical decision model with a 5-year-time horizon to follow up 1000 hypothetical patients, in order to estimate treatment costs and outcomes. Hypothetical patients, were individuals with moderate to sev...

Tacrolimus for the Treatment of Ulcerative Colitis

Tacrolimus is a calcineurin inhibitor used for the treatment of corticosteroid-refractory ulcerative colitis (UC). Two randomized controlled trials and a number of retrospective studies have assessed the therapeutic effect of tacrolimus in UC patients. These studies showed that tacrolimus has excellent short-term efficacy in corticosteroid-refractory patients, with the rates of clinical respons...

Tacrolimus Therapy for Ulcerative Colitis-Associated Post-Colectomy Enteritis

Ulcerative colitis (UC)-associated pan-enteritis is a newly identified clinical entity that occurs almost exclusively after colectomy. Characterized by diffuse small bowel mucosal inflammation not compatible with Crohn's disease, the optimal treatment modality for this condition is unknown. Tacrolimus is a potent calcineurin inhibitor that has been successfully used in the treatment of UC. We d...

A Comparison of Short- and Long-Term Therapeutic Outcomes of Infliximab- versus Tacrolimus-Based Strategies for Steroid-Refractory Ulcerative Colitis

Background/Aims. Antitumor necrosis factor antibodies and calcineurin inhibitors have shown good therapeutic efficacy for steroid-refractory ulcerative colitis (UC). Although some studies have compared the efficacy of infliximab (IFX) and cyclosporin A, there are no published studies comparing IFX and tacrolimus (Tac). This study aimed to compare therapeutic efficacies between IFX- and Tac-base...
